How long do these keep their stickiness? What is the average number I would need to purchase over a 5 year period?

Hi, thanks for yur questions. The nipples should keep the adhesive surface for quite some time. They do need to be washed each time you wear them. The come with a sample bottle of wash.
With proper care, wearing regularly they should last a year.

Can the nipples be worn in water eg swimming, Japanese Onsens etc?

Hi thank you for your question.
I would not recommend the nipples being worn swimming or in the Japanese Onsens as they temperature of the water could affect the silicone.
They also may detach during swimming. If you are wearing an Amoena swimsuit it has small pads in the pockets and therefor you would not see the nipples.
