Custom-Fit Breast Forms - Terms & Conditions

What you can expect from your Amoena Custom-fit breast form.

We want you to be comfortable knowing what you can expect during the process of scanning, fitting, and wearing an Amoena custom-fit breast form. During your scanning, you will view and sign an Expectations Agreement digitally, but we’ve provided the same information below for you to take home for your records.

I understand:

1) My fitter will conduct a mandatory bra fitting in a bra style in which I am most comfortable and wear most often. I acknowledge that I am responsible for informing her of anything related to the fit that is concerning such as style, comfort, shape, size, support, profile, etc. (NOTE: During the scanning for your Amoena Custom-fit breast form, the scanned result will mimic the shape, size and profile of your remaining breast in its supported position in the bra. The Custom-fit form will not change in size, shape, or profile once made. Please discuss any bra fit concerns with your fitter BEFORE your scanning session).

2) The Amoena Custom-fit breast form is made from polyurethane film and silicone material and therefore does not have the same appearance and feeling of real skin. The Custom-fit breast form will look like a regular breast form, but will be designed to closely match the contours of my body.

3) My Amoena Custom-fit breast form is designed to fit optimally in the bra I’m wearing during the scanning session. This is the bra style I find most comfortable and wear most often. I understand that if I wear my Amoena Custom-fit breast form with a style of bra different than the one I wore during the scan, the fit and symmetry may appear different.

4) For a unilateral patient: My Amoena Custom-fit breast form will mimic the shape, size, profile, and drape of my remaining breast in its supported position in the bra I wore during the scanning session. The size of my remaining breast can vary at any time there is a body change, such as swelling, fluid retention or weight gain or loss. I understand that my Amoena custom-fit breast form will not change in size, shape, or profile, like my remaining breast and that, if my body changes for any reason (weight loss/weight gain program, steroid usage, chemotherapy, radiotherpay, etc.) my Amoena Custom-fit breast form may not provide the same optimal fit and symmetry.

5) For a bilateral patient: My Amoena custom-fit breast form will mimic the shape and size of the breast forms and bra that I wore during the scanning session.

6) Radiotherapy can cause breast atrophy and/or sensitive skin. I confirm that I have completed radiotherapy treatment at least six months prior to my Amoena Custom-fit breast form scan and that my skin is completely healed.

7) When my Amoena Custom-fit breast form is worn against my chest wall, it may extend beyond the boundaries of my bra at the cleavage and underarm area as that is the result of the scan and final shape to replace missing tissue, and restore fullness. In this case, it is likely that the Amoena Custom-fit breast form cannot be worn in the pocket of a mastectomy bra.

8) My Amoena Custom-fit breast form has a two year warranty against manufacturing defects.

9) My Amoena Custom-fit breast form will be unique in shape and size. It will vary from any Custom-fit breast form samples shown to me or on display in the store.

I confirm:

10) That I discussed with, and notified my fitter, of any sensitive or painful areas on my chest wall, underarm, or torso, which may inhibit my ability to wear an Amoena Custom-fit breast form.

11) That I discussed with, and notified my fitter, as to whether I do or do not have a history of lymphoedema of the chest wall, arms, or torso. (NOTE: The fit of your Amoena Custom-fit breast form cannot be guaranteed if swelling is present on your chest wall, arm, torso, or surgical site).

Payment Terms & Conditions

These relate specifically to our Custom-fit breast forms, as these are bespoke items.

Payment

  • A 50%, non-refundable, deposit will be required after the scanning process takes place.
  • The remaining 50% balance will be due at your final fitting appointment.
  • Payment can be made with a debit/credit card via STPayMe link which will be sent to you from our customer service department when payment is due.

Returns & Warranty

  • Custom-fit breast forms are manufactured bespoke to each customer and therefore cannot be returned unless faulty.
  • Amoena Custom-fit breast forms have a two-year warranty against manufacturing defects.
